Care Connections
The Fairport/Perinton Senior Living Council, Inc. (SLC) partners with the Fairport Baptist Homes (FBH) to deliver the services of the Senior Options For Independence (SOFI) Care Connections.
The Senior Options For Independence (SOFI) care managers provide counseling and support for our aging population by connecting them with services they need when they need them. Follow-up services are then provided so that our seniors can remain independent and safe in their own homes as they cope with the challenges of aging. When higher levels of care are deemed necessary, the care managers facilitate the exploration of options available to our seniors. The SLC covers the salary costs of the care managers for the time they spend assisting clients of SOFI who reside within the Town of Perinton. Our belief is that, by intervening early with services, information and education, we can prevent or forestall high cost services while helping seniors maintain a better quality of life.
